Initial Tests

HBsAg - Hepatitis B surface antigen
Anti-HBc - antibody to Hepatitis B core antigen
Anti-HCV - antibody to Hepatitis C virus
Iron
TIBC - total iron binding capacity
ANA - antinuclear antibodies
SMA - smooth muscle antibodies
SPEP - serum protein electrophoresis
Ceruloplasmin
Right upper quadrant ultrasound

If the iron saturation is greater than 45%, order ferritin, HFE gene testing and consider a liver biopsy. This will evaluate for hemochromatosis.

If the ANA or SMA is positive, consider a liver biopsy.
